Inter-country MLM course for Anglophone countriestop
Pretoria, South Africa, 4 – 15 September 2006

The most important barriers to reaching every child in every district with immunisation services are related to planning and management of human, material and financial resources at  district and service delivery levels, rather than just physical barriers to access. To overcome these barriers, capacity building to improve managerial skills and to integrate the immunisation services within the social and health infrastructure is the major operational strategy. All EPI managers are therefore expected to have practical management skills to balance current collaborative efforts with achieving goals of the Immunisation Programme, which include both maintaining level and quality of routine immunisations and implementing special immunisation campaigns.

The mid-level management (MLM) course aims at providing a clear framework for public health managers, specifically those in the Immunisation Programme, so that they can better conduct their day to day managerial activities and resolve problems that arise in the achievement of their national and district EPI plans.

Results from training needs assessments conducted during the past five years(2001-2005) pointed out notably following key issues and challenges: high attrition of health staff, high turn-over, huge pre-service trainings needs, lack of sustainability of EPI training, lack of supportive supervision  and necessity of integrated training and supervision at peripheral level. The 2006 inter-country MLM courses addressed these identified needs.

The objectives were: to increase awareness of participants on roles and responsibilities of EPI managers involved in the implementation of the “Reaching every district” approach; to update/strengthen managerial skills of participants, notably those related to problem solving, effective communication, operational planning, logistics, human and financial resources management, new vaccines introduction, supplemental immunisation activities, integrated disease surveillance, monitoring and data management, supervision and immunisation services assessment.

The MLM course for Anglophone countries was organized by WHO in collaboration/ partnership with the Department of Health of South Africa, UNICEF, University of Limpopo – Medunsa Campus, USAID and NESI. Participants included professors/teachers from health training institutions, EPI managers and EPI WHO/UNICEF focal points. Participants were from the following countries: Botswana, Ethiopia, Gambia, Ghana, Kenya, Lesotho, Liberia, Malawi, Mauritius, Namibia, Nigeria, Seychelles, Sierra Leone, South Africa, Swaziland, Tanzania, Uganda, Zambia and Zimbabwe.

The workshop was facilitated by a team of experts: WHO/AFRO (6), WHO/HQ (1), UNICEF/Kenya (2), UNICEF/Zimbabwe (1), ICST/Southern Block (2), ICST/ South and East Africa (1), Medunsa Campus (4), NESI (3).

 

Inter-country Vaccinology Workshoptop
Dakar, Senegal, 9 – 11 August 2006

The aim of this workshop organised by GSK, was to update participants on new vaccine developments and to provide a platform to discuss practical issues regarding EPI programme implementation. Invited participants included EPI managers from Senegal, Mali, Cote d´Ivoire and Burkina Faso, and professors/teachers from universities and nursing schools. The overall feedback from participants regarding the usefulness and content of the workshop was very positive, as all participants mentioned that they had learned new aspects or components on vaccination and benefited from networking with colleagues. The main recommendations by the participants included: (1) there is a major need to increase the capacity of medical schools and nursing training institutions to improve their capacity for training on vaccinations; (2) more local experts should be included in facilitating of the sessions; and (3) there is a need to ensure external funding of vaccination programmes beyond current period of donor commitment to ensure continuity, while local government should also mobilise additional resources.

Twenty countries were represented at the course with a total of 58 participants: Algeria (3), Benin (12), Burkina Faso (2), Burundi (2), Cameroon (2), Comoros (2), Congo-Brazzaville (2), Cote d´Ivoire (2), Congo-Kinshasa-RDC (3), Gabon (3), Guinée (2), Mali (2), Madagascar (1), Mauritania (2), Niger (4), République Centrafricaine (3), Rwanda (2), Senegal (2), Tchad (5), Togo (2).

The workshop was facilitated by a team of experts (17): WHO-ICP West (4), WHO-ICP Central (3), WHO-AFRO Region Harare (3), WHO-HQ Geneva (1), UNICEF Western Africa Office (3), Agence de Médecine Preventive (2) and NESI (1).

Vaccinology/value of vaccination workshoptop
Kampala, Uganda, 30 May – 1 June 2006

The inter-country workshop on vaccinology and value of vaccination was organized by the Ministry of Health of Uganda, in collaboration with NESI and supported by an unrestricted educational grant from GSK. The objectives of the workshop were to introduce participants to the various disciplines associated with vaccinology including immunology, clinical trials methods, new vaccines, vaccine safety, vaccination strategies, evaluation of immunisation programmes and vaccine economics. During the last session of the workshop, special attention was given to the value of vaccination. The workshop was attended by 28 participants from Uganda, Kenya and Tanzania. The speakers panel consisted of experts from MoH Uganda, WHO, UNICEF, WorldBank, National Drug Authority, DANIDA and NESI.

Consensus workshop on EPI curricula prototypes for medical and nursing/midwifery schools in the WHO African Region top
Douala, Cameroon, 13 – 17 March 2006

A recent report on external evaluation of the EPI management training recommended to encourage countries to link professional education and academics/teachers with service realities harmonising pre-service and in-service training. The report further suggested that an EPI generic curriculum for pre-service institutions should be developed and disseminated through workshops and seminars at country level. There has therefore been a collective opinion that a systematic revision of EPI curriculum for both medical and nursing/midwifery schools should be carried out. WHO/AFRO and NESI in collaboration with UNICEF drafted two EPI curricula prototypes for medical and nursing/midwifery schools.

Accordingly a workshop was organised for professors/teachers to get a consensus on the two curricula prototypes, and to make recommendations for the implementation of the EPI curricula revision process within the African health pre-service training institutions. Prior to the actual workshop, two days for training of trainers (ToT) were organised. The workshop was conducted in two official languages of the WHO African Region (English and French) with simultaneous translation. A total number of 49 participants from 21 countries attended the workshop. The participants were Heads/Training Coordinators of Colleges/ Faculties of Medicine/Nursing and University departments (13), professors (8), teachers and tutors (20) as well as representatives of Ministries of Health (6) and partner organisations (2). Participating countries were: Benin, Burkina Faso, Burundi, Cameroon, Central African Republic, Chad, Congo/Brazzaville, Cote d´Ivoire, DRC, Ethiopia, Ghana, Kenya, Madagascar, Mali, Niger, Senegal, Tanzania, Togo, Uganda, Zambia and Zimbabwe. The workshop was facilitated by a pool of experts (11) from WHO/AFRO (2), WHO Inter-country team (3) and Country Offices (1), UNICEF Regional Office (1), MEDUNSA University of South Africa (1), AMP (1) and NESI consultants (2). A daily formative evaluation was conducted regarding object achievement, content relevance, effectiveness of facilitation and participation, and organisation. A summative evaluation was conducted at the end.

East Africa Inter-country Data Quality Self-Assessment (DQS) Workshoptop
Nairobi, Kenya, 6-13 March 2006

A ´Training of trainers´ DQS workshop was organised (WHO-AFRO and NESI) for national and regional participants to learn how to perform a DQS to obtain country-specific results on routine practices that can be used to continually monitor and improve the practice and management of immunisation activities at the district level. DQS can identify districts/health facilities with low quality or inaccurate immunisation monitoring, identify reasons for poor quality monitoring, identify bottlenecks of information flow, raise awareness of use of data for planning immunisation at local and district levels and is an opportunity to provide a refresher on good practices. The objectives of the workshop are: learn how to conduct a DQS and its purpose in the context of the RED approach, review current monitoring practices at the country level and how to design a country-specific self-assessment tool, conduct a practical assessment using the generic DQS tool to diagnose a monitoring system, analyse the results from a DQS and learn how to develop conclusions and recommendations for a specific country, and develop a country-specific plan.

The workshop was attended by 20 participants from 6 countries (Kenya, Tanzania, Uganda, Rwanda, Ethiopia and Eritrea). The workshop was facilitated by a pool of experts: WHO/ICP EPI EA (4), WHO EPI Kenya (1), WHO/AFRO (2), WHO ICP/SA (1) and NESI (1).

Vaccinology/value of vaccination workshop top
Rabat, Morocco, 4 – 6 December 2006

The workshop was organised by the Ministry of Health of Morocco, GSK and NESI. The aim of the workshop was to bring together participants from the Maghreb region to exchange opinions and experiences in the context of updated information on international vaccine initiatives, scientific information on new vaccine developments, latest insights regarding the value of vaccination and the importance of advocacy for vaccination. The meeting also provided a platform for participants to discuss practical issues regarding EPI programme implementation, the latest vaccination schedules recommended by WHO/EMRO, vaccine safety, surveillance for programme planning, evaluation and pharmaco-vigilance. Three working groups looked into three specific topics: registration and regulation of vaccines, training on different aspects of vaccines and immunisation and the role of the public and private sector in ensuring to minimise the incidence of vaccine preventable diseases in the respective countries and the region. The workshop was attended by 40 participants from 3 countries: Algeria, Morocco and Tunisia. The workshop was facilitated by a pool of experts: MoH (4), Universities (4), GAVI (1), GSK (1), consultant (1) and NESI (2).


Education and training activities in 2005

Inter-country Mid-Level Management (MLM) Coursetop
Lusaka, Zambia, 1-12 August 2005

The 13th inter-country training course on the Expanded Programme on Immunisation (EPI) Mid-Level Management was organised for English-speaking African countries in the Eastern, Western, and Southern epidemiological blocks.

The general objective of this MLM course was to manage immunisation services within the context of GIVS, the 2001-2005 EPI Regional Strategic Plan, and the RED strategy. The 2005 inter-country courses provided an opportunity to update all facilitators and participants on GIVS. Most importantly, participants were trained on how to use the guidelines to develop/revise a multi-year plan (MYP) in line with GIVS. Two newly developed MLM modules were also tested during this inter-country course.

A total of 78 participants attended the course from the following 20 countries: Botswana, Gambia, Ghana, Sierra Leone, Nigeria, Eritrea, Ethiopia, Kenya, Lesotho, Namibia, Malawi, Mauritius, Uganda, Seychelles, South Africa, Swaziland, Tanzania, Zimbabwe, and Zambia. Among these participants, there were 44 district/provincial/national EPI managers, 10 professors, and 24 WHO/UNICEF/NESI and other partners.

NESI provided both technical assistance and a significant financial contribution to this training course. NESI staff acted as facilitators, using active coaching and teaching with participants to help them to achieve their learning objectives. NESI’s financial contribution enabled academic experts to attend the workshop.

Developing vaccinology expertise for Sub-Saharan Africatop
Cape Town, South Africa, 26 June - 1 July 2005

This vaccinology course was organised by the University of Cape Town (UCT - Institute of Infectious Disease and molecular Medicine) in collaboration with tropEd, Charité, the University of Antwerpen, GSK and NESI.

The course was intended for clinicians, public health practitioners and scientists in Sub-Saharan Africa who are involved in any aspect of vaccinology from development through implementation and evaluation. The goal of this course was to introduce participants to the various disciplines associated with vaccinology including vaccine development, the relevant basic sciences such as immunology, microbiology, epidemiology, clinical trials methods, vaccine safety, vaccine economics, vaccination strategy and evaluation of vaccines. For researchers and scientists the main objective was to build sustainable research capacity related to vaccine development and/or conduct standard phase 1 to phase 4 vaccine trials. For clinicians and public health specialists the main objective was to provide them with the necessary expertise to support public vaccination programmes and related activities.

Inter-country EPI Mid-Level Management (MLM) Coursetop
Ouidah, Benin, 23 May-3 June 2005

The inter-country course for French-speaking African countries was hosted by the Republic of Benin and organised by WHO/AFRO in collaboration with UNICEF, CATR/ARIVA, UNF, USAID, NESI, and other GAVI Partners.

This MLM inter-country course was attended by national, provincial/district EPI managers, teachers/professors and WHO/UNICEF EPI focal persons coming from the following 20 countries: Algeria, Benin, Burkina Faso, Burundi, Cameroon, Comoros, Congo, Cote d’Ivoire, Gabon, Guinea, Madagascar, Mauritania, Mali, Niger, Central African Republic, Democratic Republic of Congo, Rwanda, Senegal, Chad, and Togo. The course was facilitated by a pool of experts: WHO/AFR (8), WHO/ICP/West (2), WHO/ICP/Central (3), WHO/HQ (2), UNICEF/WCARO (2), EPI Benin (1), ARIVA (1), University Cotonou (1) and NESI (2).

As each MLM course this course comprised of a two-days session for training of trainers (ToT) and eleven days for EPI managers/teachers session. The aim of this course was to enable participants to manage immunisation services within the context of the Global Immunisation Vision and Strategy (GIVS), the 2001-2005 EPI Regional Strategic Plan, and the RED strategy. This course was also used to validate the content and to test two new MLM modules on Transport management and Financing.

NESI provided both technical assistance and a significant financial contribution to this training course. NESI staff acted as facilitators using active coaching and teaching with participants to help them to achieve their learning objectives. NESI’s financial contribution was used to cover the expenses of the training materials and to invite academic experts. NESI’s specific contribution, namely to invite key academics from nursing and medical schools was recognised as an important contribution toward strengthening the pre-service capacity building for EPI.

Saly/Dakar, Senegal, 5 - 7 November 2003

NESI organised, in collaboration with WHO/AFRO and other GAVI partners, its first meeting for French-speaking countries in West Africa. NESI had the lead role in establishing the scientific programme, selecting the speakers, choosing the venue and inviting the academic staff. NESI also provided administrative support during the meeting.

NESI provided both technical assistance and a significant financial contribution to this event. NESI staff acted as facilitators using active coaching and teaching with participants to help them achieve their learning objectives. The financial contribution was used to invite 13 academic experts.

NESI provided both technical assistance and a significant financial contribution to this event. NESI staff co-facilitated the course with representatives from WHO, UNICEF, and the The Vaccine Fund. The facilitators used a number of active coaching and teaching methods with participants to help them achieve their learning objectives. NESI’s financial contribution was used to invite 11 academic experts from 9 countries to the course.

Sharm El Sheikh, Egypt, 26 - 29 May 2003

This workshop was organised by WHO Regional Office for the Eastern Mediterranean (WHO/EMRO) and UNICEF Middle East and North Africa Regional Office (UNICEF/MENARO), in collaboration with NESI. Technical support was provided by Carsten Mantel, University of Berlin, and NESI staff, who served as facilitators for the working group sessions. NESI also provided financial support by paying for the meeting facilities.
